Joanne Fadale Wagner, MSW, LICSW, CCM has been the Social Worker Care Manager with the Dartmouth Health System HTC for 14 years. Joanne is currently on the Bleeding Disorders Substance Use and Mental Health Advisory Council and was a member of the NHF Social Work Working Group for 4 years. Joanne’s past experience on the New Hampshire’s Advisory Panel for Cancer and Chronic Disease has helped her in advocating for patient needs. Joanne has over 35 years of experience in social work and as a mental health clinician. She is experienced in both health care and social service administration, as well as clinically in the areas of medical and behavioral health care, chronic care management and geriatrics. Joanne is a State Approved LICSW Supervisor and MSW Student Field Supervisor and works to advocate for patients/families and the profession.
