She was accepted into a facility, which later denied her after learning she had a bleeding disorder. I spoke with the emergency department care management team and assisted them in contacting the facility. I utilized the provider toolkit created by BD SUMHAC to discuss and educate the facility about bleeding disorders. The toolkit provides specific language that helps reinforce to facilities that patients with bleeding disorders are medically stable. This patient was not on prophylaxis and did not have any active bleeding symptoms. I was also able to have the facility provider speak with our pediatric hematologist. After the doctor-to-doctor call, the patient was accepted into the facility and admitted that day.
